We strive to improve the health of the community we serve by providing comprehensive health prevention services.
We provide physical activity and healthy eating interventions tailored to the cultural identity, specific needs, and preferential interests of the community. We offer an important opportunity to promote population health through systematically addressing the social determinants of health and monitoring the basic unmet needs of vulnerable adults and linking them to community services.
GAD coordinates the L.I.V.E. Club that provides a welcoming environment in which community members build a culture of health, learn healthy eating tips and taste healthy foods, network with others facing similar health issues, and hear about new and effective chronic disease management strategies. The model extends standard counseling by doctors to include giving participants cultural and language sensitive and easy to understand education materials.
